Both Jack Nicholson and Morgan Freeman actually shaved their heads for this movie.
Justin Zackham said that he wrote the script with Morgan Freeman in mind and managed to finish it in just 2 weeks.
Jack Nicholson had a recent stint in the hospital just prior to filming this movie. As a result some of the dialogue in the hospital comes from his own experiences while there. The mirror-glasses were also a personal possession of Nicholson's from the same hospital stay. He brought them in and showed the to Rob Reiner who agreed to use them in the film.
Alfonso Freeman who plays Morgan Freeman's son in this film, is the real-life son of the famous actor.
